Willows Health and Fitness Crèche – Terms and Conditions

Welcome to the Willows Crèche

At Willows Health and Fitness, we believe that supporting your wellbeing means caring for your whole family. Our crèche is designed to create a safe, nurturing, and engaging environment for your little ones, giving you peace of mind while you focus on your health and happiness.

By using our crèche services, you are agreeing to the following terms and conditions, which ensure the safety, comfort, and wellbeing of all families and staff in our community.

  1. Age Eligibility

Our crèche warmly welcomes infants and children aged 10 weeks to 5 years. Children over 5 years of age are not permitted to attend the crèche.

  1. Health and Wellbeing

Your child's health is our priority. Please ensure that:

  • Children who are unwell (e.g., fever, vomiting, diarrhoea, rashes, contagious conditions) must not attend the crèche.
  • Staff reserve the right to refuse entry if a child appears unwell upon arrival.
  • Should your child become unwell during their session, you will be contacted immediately and asked to collect them promptly.

We appreciate your partnership in keeping our community healthy.

  1. Bookings and Attendance

To maintain a calm and welcoming environment:

  • Bookings are essential to secure your child's place.
  • Sessions are limited to a maximum of 60 minutes per child per visit (9-10am or 10-11am).
  • Parents/guardians must remain on the Willows Health and Fitness premises at all times while their child is in the crèche.
  • Only authorised persons (as nominated by the parent/guardian) are permitted to collect a child.
  1. Arrival and Departure
  • Please sign your child in and out of each session at the crèche reception. Children must be collected by the agreed end time of the session (10am or 11am).

We appreciate your understanding as timely pickups help us care for all families smoothly.

  1. Fees
  • $5 per week per child for unlimited access as an add-on to your membership. Payment is direct debited as part of your weekly fees.
  • or $5 per child per session pay-as-you-go.
  • All pay-as-you-go fees must be settled prior to or at the time of your child's session.

 

  1. What to Bring

We encourage parents to provide:

  • Nappies, wipes, and a change of clothes (if applicable)
  • Bottles/drinks (clearly labelled)

Please note: No food is permitted to be brought or consumed in the crèche area. This helps prevent allergic reactions and reduces the risk of choking, and aligns with our role as a short-term supervised space, not a daycare service.

All personal items should be labelled to avoid loss.

Please note: Willows staff are unable to administer medications.

  1. Behaviour and Safety

At Willows, we foster a caring and respectful environment.

  • Positive behaviour is encouraged and modelled.
  • In the rare instance that a child's behaviour risks the safety of others, parents may be called to collect their child early.
  • Repeated unsafe behaviour may result in suspension from crèche services.

We work together to build a happy, safe space for every child.

  1. Emergency and Medical Care

In the event of an emergency, parents will be contacted immediately. Should urgent medical care be required, an ambulance will be called. Associated costs will be the responsibility of the parent/guardian.

Please ensure your contact details are always up to date with Willows Reception.

  1. Privacy and Confidentiality

Your family's privacy matters to us. Personal information is securely stored and used only for crèche operations. No photos or videos will be taken without prior consent.

  1. Acknowledgement and Agreement

All parents and guardians must read, understand, and sign this agreement before their child can attend the crèche.

By signing, you are acknowledging your understanding of these terms and your commitment to supporting our community's shared wellbeing.

Release and Indemnity In consideration of the acceptance of my payment for participating in the activity (and except to the extent that the same may be precluded by state) I agree to release and indemnify Willows Health and Lifestyle Centre as follows: • I participate in the activity at my sole risk and responsibility • I release, indemnify and hold harmless Willows Health and Lifestyle Centre, its servants and agents, from and against all and any actions or claims which may be made by me or on my behalf or by other parties for or in respect of or arising out of any injury, loss, damage, or death caused to me or my property whether by negligence, breach of contract or in any way whatsoever. I also agree that in the event that I am injured or my property is damaged, I will bring no claim, legal or otherwise, against Willows Health and Lifestyle Centre in respect of that injury or damage. Where Participant is under 18 years of age: I being the parent/guardian of the person named in the acknowledgement and release hereby acknowledge and agree that: • I have read the whole of this document and understand it. • I consent to the person named in this acknowledgement and release participating in the activity and • I am aware of the risks, dangers and obligations set out above in the acknowledgement and release. In Consideration of the person named in the Acknowledgement and Release being accepted to participate in the activity. I agree to release and indemnify Willows Health & Lifestyle Centre in the same manner and to the same effect and extent as if I were the person first named in this Acknowledgement and Release and the person participating in the activity.

1. Creche Registration Form

Please complete the following form below for your child/ren.

MEDICAL AND HEALTH INFORMATION:
Does the child have any disabilities or medical conditions? If yes, please provide details.
Does the child usually require regular medication or special aids (e.g. glasses, hearing aid etc.)? If yes, please provide details.
Does your child have any allergies (to foods, antiobiotics, insects, plants, creams etc)? If yes, please provide details (also list what type of reaction they have to these).
Is there any other medical information we may need to know? If yes, please provide details.
